Health information and your privacy

Your dental record is health information. In Alberta that is governed by the Health Information

Act, and it carries obligations the practice takes seriously. This page explains what those are

in plain language.

It is separate from our Privacy Policy, which covers the website only.

Heritage Dental Centre is a custodian

Under the Health Information Act, dentists are custodians of health information. That is a defined

role with duties attached, not a description. It means the practice is responsible for what it

collects about you, how it is used, who it goes to, and how it is protected.

It also applies to more than treatment notes. Your name, address, phone number and health care

number are health information too, because of who holds them and why.

To identify you and reach you: your name, contact details, date of birth and health care

number.

To treat you safely: your dental and relevant medical history, medications, allergies,

examination findings, x-rays and photographs, and notes of what was done and what was discussed.

The practice collects what it needs for those purposes and does not collect information it has no

use for.

Your insurer, when you ask us to bill them. What is sent is what the claim requires.
Another health provider, on referral. If you are sent to a specialist, the information they

need to treat you goes with you.

Where the law requires it. Some disclosures are mandatory, for example a court order.

Your record is not sold, and it is not used for marketing. Nothing from your dental record is

sent to Google, to Meta or to any advertising platform. This website does measure which of its

pages are visited, including pages about particular treatments; our Privacy Policy

explains exactly what that records and how to limit it.

Under the Act you can:

Ask to see your own record, and get a copy of it.
Ask for a correction if something in it is wrong.
Ask who it has been given to, and why.
Complain, to the practice or to the Office of the Information and Privacy Commissioner of

Alberta, if you think your information has been mishandled.

If you would like your records or x-rays sent to another dental office, we ask you to sign a records release form first.

To make any of these requests, call the practice on 780-434-8749 and say what you are asking for.

Records are kept for as long as the law and the practice's professional obligations require, and

disposed of securely after that.

If health information is lost, or somebody gets access to it who should not have, the Act requires

the practice to give notice where there is a risk of harm. That is an obligation, not a courtesy.
